If you have inquiries on your report, you CAN dispute them with each bureau but the chances are they're going to tell you to contact the bank/company that requested your credit.
As for changing your name, yes you can. It's not a 1 step process though. As for changing your SSN, you can only do this 1 way. With documents, you will have to prove to and convince the social security office that your case of identity theft is so severe that it cannot be resolved, and you'll also have to show and convince them that you've done everything possible trying to fix your ID theft.EQUIFAX my credit report?

What you need to do is write a letter to them called a dispute. Say you never made application for that company or the service. Then you have to forward copies to the other 2 agencies. It will be removed as you never made the transaction.
